Write  your favorite sports quote and tell who said it and what it means to you.

“Success is no accident. It is hard work, perseverance, learning, studying, sacrifice and most of all, love of what you are doing or learning to do."
Above is a quote by Pele, probably the greatest soccer player who ever lived. It's very meaningful to me. It tells me that if I work hard and strive for success, then I'll have a chance at earning success. To me, that makes me strive to find my passions and work hard at what I love. This quote gives me a sense of purpose that I believe will make me become a better person.
What does wellness mean to you?

To me, wellness means to make good choices towards a healthy life. It means to strive to fulfill deeds that will make your life better.

What do you do on a daily basis to be well and healthy?

About every other day, my friend and I play soccer after school.

BRAIN BREAK: 20 Jumping Jacks
Rate your enjoyment of  PE? 1-5 (1=poor 5=awesome)
What are your favorite things about PE?

I would rate my enjoyment of PE a 4. One of my favorite things about PE are when we get to a sport that I have knowledge about. Also, when we go from sport to sport, I love when I get to a sport that I didn't think I would like at first but then end up liking it. It gives me a good feeling inside when I find a new passion in PE.

Rate your enjoyment of  Health? 1-5 (1=poor 5=awesome)
What are your favorite things about Health?

I would rate my enjoyment of Health a 3. I like Health because every once in a while, I'll learn something interesting that I didn't know before, and a lot of the times when that happens, I find later that what I learned in Health affects my daily life. It's cool to see how you can connect a subject in school with everyday life.

What would you change about the Health or PE program?

In Health, I would change the amount of computer work that we do. I would much rather be out experiencing what we learn outside rather than on a computer screen. In PE, I would change the amount of days we have to complete a PE unit. I think that if we were to love every PE unit more, we should spend more time on it, so we can work on the unit a little longer.

BRAIN BREAK: 1 minute of push-ups (or as many as you can do in 1 minute)
How physically active have you been this year on a weekly basis? Share as many of the sports, select teams, intramural sports or activities that you participated in this year in 6th grade. Include activities you do both in and out of school.

I believe that this year, I've been pretty physically active. I think this because I usually go out and play soccer after school, I played on the SAS A team for soccer, I played in the soccer intramural, and I played in a soccer league outside of school called ANZA. As you may have noticed, basically all of the sports activities that I've done this year have been soccer. This could mean that I should try other sports.

Finish this sentence. I am a good teammate because…

I know that no matter what position or ranking I am on a team, I'm nothing without my teammates. I know that we all affect the team equally and that no single player is more special than the team as a whole.

How have you shown leadership this year in PE? What qualities make you a good leader?
(If you have not taken the opportunity to lead this year what could you do to improve on this next year?)

I believe that I have shown leadership this year in PE, but I should have done more to become known as a leader among my PE schoolmates. Next year, I will try harder to become more of a leader and help organize and move along my schoolmates.

What is your strongest area of physical fitness? (strength, flexibility, cardio respiratory endurance) Which area do you need to work on?

Cardio respiratory endurance because almost every time I run a KM, I try harder and harder to improve.

Why do you think being physically active is important?

It's important because if you aren't physically active, you could get diabetes or other bad diseases and your self esteem could downgrade.

BRAIN BREAK: One minute of sit-ups
Describe your strengths and weaknesses eating ‘MyPlate’ 3 meals a day.

My strength is that when my mom tells me to try something new that she says is good for me, I usually eat it. My weakness is that I've been eating to many sweets lately, and I need to cut down the amount of sweets I eat now.

What is your food goal? (Review your MyPlate blog post) Did you achieve your “food goal” or do you need to continue to work on it?

I think I have achieved that goal.

How stressed have you been this semester?

Not very stressed.

What two strategies work best for you to reduce stress?

Hanging out with friends and playing soccer or running.

BRAIN BREAK: Hold the hamstring stretch for 30 seconds x2
If you finish early and are proud of your work, you may watch the following videos with your ear buds in.
